Initial Evaluation - $300

Follow-up Appointments - $135

*A non-refundable deposit of $150 is required to schedule an appointment as a new patient. This deposit will be applied towards your total initial evaluation fee ($150 collected at time of booking, $150 collected at time of appointment)

*As an out-of-network provider, I do not accept insurance. Acceptable payment forms include cash, credit card, check, HSA card. You may use your insurance at the pharmacy for medications/prescriptions and labs.
